Puerto Rico's head of state is the President of the United States. People born in Puerto Rico are U.S. citizens. As such, they are entitled to vote at the federal level, but not from the island, as the territory is not yet incorporated - but yes, they do vote.

Do you need a passport to travel there? No. You going there, and vice versa, is just like going to Hawaii - no passport necessary.
